Case Managers
University Hospitals has hired case managers to assist and facilitate the care of inpatients. Case managers will:
·
Obtain outside hospital and nursing home records
·
Assist in contacting families and coordinating family meetings
·
Participate in family meetings
·
Schedule antibiotic administration at Chagrin Highlands
·
Order durable medical equipment (DME)
·
Facilitate completion of consults to prevent delay in discharge
· Facilitate PICC line placements by communicating with PICC line nurses and interventional radiology
